Certified Diabetes Educator

Provide all aspects of diabetes education to adolescent and adult patients with type 1, type 2, and gestational diabetes, as well as pre-diabetes, in group and/or individual setting. Collaborate with referring provider to recommend, initiate and adjust medications as needed. Provide Medical Nutrition Therapy (MNT) for patients on individual basis for other medical conditions (if position filled by RD). Instructor for ACT lifestyle and nutrition intervention program for preventing and managing chronic disease. Participate in department projects and quality improvement initiatives.
- Registered Dietitian (RD) or Registered Nurse (RN)
- Certified Diabetes Educator (CDE), or eligible within 6 months
- Experience with insulin pumps and continuous glucose monitors
- Experience with diabetes medication management preferred
